Why it matters: CalOptima Health is spending reserves to hold its network together while operating margins turn negative and membership shrinks. The $429.6 million commitment runs through December 2028, overlapping the MCO tax replacement and directed-payment phase-down, so providers negotiating with the plan should read it against those timelines.

Why it matters: Losing about a third of membership in one budget year changes the plan's revenue base, its delegation economics with VHP, and the size of its CalAIM programs. Retention work depends on county renewal data sharing, and that exchange is still being planned.

Why it matters: Two consecutive loss months at 97-98% MLR mark a turn from a year that closed with $62.4 million in net income. The capital cushion is large, but pharmacy unit cost and rising cost concentration (13.1% of members driving 89.8% of costs) point to pressure that reserves absorb rather than solve.
